(LAKEWOOD RANCH, FL) – Mustangs must attend their homerooms before first period Thursday to receive their 2022-23 Bio-forms.

Bio-forms must be completed so the school has the correct contact information such as parent phone numbers for calls related to health, attendance, and other emergencies.

“These forms are important because in case of emergency the school has a way to contact student’s parents,” said LRHS assistant principal Thomas Bellantonio.

Forms must be turned in to Mustangs’ first period teacher by Monday, Sept. 26.

If forms have information filled out from previous years, only the incorrect information must be updated to show correct information.

If the forms have all the correct information, then they do not need to be turned in again.
