(LAKEWOOD RANCH, FL)- On the first day of school every student was given both biographical and health information forms. These forms are to be filled out and returned to their first period teacher as soon as possible.
Biographical forms were given out by the registration office and contain information such as a student’s address and emergency contacts. They contain confidential information that the registration office needs.
“The bio forms are very important as in the cause of emergency we need the information on it to contacts your parents or guardian,” said LRHS registrar Leota Walker.
Students were also given health forms that the nurses use so they know if the student has any allergies or other medical conditions.
“It is important that students return these forms ASAP,” said clinic aide Laura Anderson. “This is because we need them to take care of our students and make sure they are healthy and ready to learn.”
